I can't get this to post a follow-up, so here's my input... :-)

Nicole -

I am in about the same boat as you, and would really appreciate your willingness to share any info with me!

As for addition sections to include:
-practice expectations and what happens if the girls fail to meet them
(missing part of uniform - they dont' march)
-financial expecations - I wrote these out for the girls before they auditioned
-parental expectations - I tell mine I expect them all to help in SOME way - that could be sewing flags, building backdrops, letting us use their house for a spaghetti dinner (i do this every year but my apartment is too small to host it), wash, dry and fold a batch of flags... that way the parents know they're responsible for being involved.
-provide index cards for all parents with your contact info
-get the emergency contact info from parents at this meeting
-work with parents to establish high scholastic goals for their children while they're involved
