Color Guard handbook? need input please!

As the new Color Gurad instructor, I am holding tryouts for the first time. Tryouts will be the first week in April, and will consist of the following:

first half of school song
marching and spinning at the same time
small dance routine to show ease of body movement
part of a hand routine we use in the stands to show how fast they can learn intricate hand movements.
Short flag routine to music (so the older members have some flag stuff to learn too. THis will be important in weeding out the weaker ones that did NOT improve with practice.)

There will be an informal meeting after tryouts for the members and their parents. At this meeting, many things will be addressed but I am wanting to come up with a small handout that outlines the expectations and rules of being in the color guard, but I come up with only a few items.

I also want the parents to sign a form saying that they agree to the rules that are set in place, and agree to help their child uphold those expectations.

Does anyone have a sample of such information that they can email me?

I am having trouble with wording and thinking of EVERY situation that may arise, so I am prepared.
