We are a small private HS that unfornuately did not have cheerleaders this year due to a lack of interest. This of course was very noticable. We have an opportunity to change this for the next school year. I have been asked to develop an "Interest Form" to distribute to the student body. The form should include information regarding costs, responsibilites, etc. I'm wondering if anyone has a form such as this that I could tailor to fit our school. If you wouldn't mind sharing I would greatly appreciate it.
